I have lost my insurance policy document. Can I get a duplicate copy?

It is unfortunate that you have lost your insurance policy document. However, you need not worry because you can apply for a duplicate copy in a prescribed form depending from which insurer you bought the policy in the first place. Insurers usually charge a nominal fee to issue a duplicate copy of the policy. Also, you should submit proofs of your identity, the premium receipt, the policy number and a photocopy of the original policy (not mandatory) and an indemnity bond. If the policy document is torn or partially destroyed due to certain reason, the rest of the document, which remains, should be returned to the insurer as evidence.
