On which grounds the cashless benefit under my health insurance can be declined?

I have a health insurance policy, but I was declined the cashless benefit by the hospital in which I was admitted. On what circumstances are such benefits declined?

The cashless facility under a health policy is provided by most insurers by making an arrangement with various hospitals. Therefore, it purely depends on the arrangement of the insurer with a particular hospital. Generally, insurers cover all major hospitals, but looking at the number of hospitals, it may not be possible for any insurer to cover all of them. It might be that the hospital in which you received the treatment was not covered under such an arrangement. You can, however, file a claim enclosing all the documents relating to illness and hospitalisation, as covered under the policy. The insurers will reimburse the expenditure.
